7/31/2023 EnchantaVaughn Approved Behavior: Client actively participated with QMHP as they discussed and processed empathy. Client appeared to be learning from her mistakes and correcting them while QMHP went over her treatment plan with her mom and discussed important information. Client recognized her triggers and was able to identify common interests and similarities her and other ADHD recipients share. Client demonstrated responsibility and leadership. Client was acceptive of redirection. Client showed interest in building interpersonal relationships with peers and authority to increase support system. Behavior: Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Client demonstrated that she was able to process productive expressions of anger. Client displayed active behaviors in group settings. Client is making progress with expressing herself. Client made insightful comments which contributed to discussion. Client learned to avoid negative and defiant behavior and comply to rules. Client was seen following directions. Client was observed in session utilizing and learning new interventions and strategies with her family. Client engaged in self-reflection, discussing good and bad parts of her day. Intervention: QMHP went to the home for the purpose of meeting with client individually to discuss client’s behaviors in the home. QMHP engaged in conversation with client about her day. QMHP administered therapy session by exploring the underlying factors contributing to the client’s pattern of blaming others for her disruptive behaviors and negative attitude. QMHP listened to client verbalize her feelings towards making good decisions at home and avoiding conflicts with mom and other adult family members. QMHP encouraged client to have a good day. Intervention: QMHP monitored individualized behaviors for acquisition of objectives. QMHP encouraged client to listen to her mother. QMHP encouraged client to be the example. QMHP firmly and consistently confronted client on how her involvement with negative peer group impacts herself and others. QMHP utilized a role-reversal technique to assist client with alternative ways to avoid negative interaction and to assist her with better decision-making skills. QMHP supported client as she identified how her patterns of blaming others is associated with underlying feeling of inadequacy and insecurity. . Response: Client transitions smoothly and safely to assigned area in the home, displaying appropriate behaviors with mom and QMHP. Client communicated thoughts and feelings well during session. Client made constant direct eye contact with authority figures. Mom states to counselor that she has implemented a minor schedule for client to assist her with her completion of chores. Mom mentions that schedule assists her as well and allows her to remain focused on completing tasks and chores concerning client. Response: Client was pleased to be praised for her behaviors. Client stated that she felt she was smart and kind to her family. Client communicated that it is often hard to remain positive at all times about herself when she doesn’t always have the best things or necessities that she needs, especially since not being able to go anywhere or do anything. Client discussed numerous situations where she had to adapt even when she didn’t want to. Client expressed that she tries to think highly of herself. Client communicated that she feels that she will be more successful, due to having a woman as a counselor.
